Change of Address
Must be completed by residents of the 1st Municipal District who wish to change their mailing addresses.
Affidavit for Continuation of Homestead Exemption
Residents of the 1st Municipal District who are not currently residing in their damaged homes, and do not have trailers on the property of their damaged homes must complete this affidavit, have it notarized, and turn submit it along with their Homestead Exemption card if they wish to continue their Homestead Exemption on the property.

Special Assessment Application
Must be completed by all residents of the 1st Municipal District who meet the criteria stated in the LTC Special Assessment Level Primer and who wish to apply for the Special Assessment Level Primer.
